Probably a lazy shift from 4th to 5th.

 

Keep an eye on it, if it keeps happening, you'll probably have to service the rider. You might want to look at your riding boots. Too soft a top can make for sloppy shifting. As you run up through the gears, the tendency is to move the foot around on the pegs as it recovers from being on the ground. Your normal foot position might just be unsuitable for proper shifting.

 

Be deliberate about placement of your foot so that your toe/arch junction is directly under the lever and then execute the shift like you should all, quickly and positively. Your problems should be gone.

i was advised by this board to keep a "load" on the shift until the shift is complete. that is keep a bit of pressure on as you upshift. not much and it does becomes 2nd nature.
