I never like reading about loss of life esp. in this matter.  Senseless...

On the other hand Massachusetts seems to be everything in a timely fashion...

I suppose it will be another six or seven years before they get up to 2019.

"The latest round of suspensions, outlined in an interim progress report, were found in the bins at the Quincy headquarters as well as an archives facility in Concord, Massachusetts, where boxes of out-of-state violations from 2011 to 2017 were pulled".

Seems to be more than mere negligence, but reckless and/or intentional dereliction of duty.  I will be interested to see if a prosecutor goes after the former head of this agency and key subordinates who made the decision not to process these violations.
